- Review the Nomination Guidelines.
- The lifesaving incident must have occurred between June 1, 2020 and May 31, 2021.
- Nominations must be received no later than June 30, 2021.
- Submit the nomination:
Complete the nomination online below, OR
Print, fill out the Application for Lifesaving Award, scan it and upload to this web portal.
The online form can be started and saved for completion later as needed.
Photo Releases can be submitted with this nomination or submitted later upon notification of acceptance of the nomination.
Printed and signed application forms may also be submitted by US Mail to the address at the bottom of this page.
